School Forms Online
Student forms are now available to SBS parents on School Forms Online (SFO) for our 2019-20 school year.
Please look for an email from “firstname.lastname@example.org” which will include login information and a link to SFO. If you are a returning parent and have not received this email, please check your spam filter, and contact email@example.com with any questions. Once you log in you will see a home screen, customized for you and your SBS student(s):
You will see at least two accounts on the left: one with your student’s name and number of forms to complete; the other, called “Family,” with number of forms to complete. If you have more than one child attending SBS, each will have a separate set of forms listed under her name for completion in addition to the family account. If you are a non-custodial parent, you will see a smaller set of forms. If needed, please work with the parent listed as “primary” on your school application to complete forms together.Click on each icon to see the forms and information assigned, and the due dates for submission.Most forms can be filled out and submitted online. Once you have completed and submitted your form, you will receive a PDF link, date and time-stamped, as confirmation and the form will be marked complete in your home screen.
Non-SBS forms still require printing and submission via mail or email. Mail completed forms to: Stoneleigh-Burnham School, attn: Shelley Futter, 574 Bernardston Road, Greenfield, MA 01301 or you may email scans/PDFs to: firstname.lastname@example.org.Some “forms” are informational pieces that do not need to be submitted. Important information from the Health Center: the Authorization for Medical Surgical Treatment MUST be submitted by July 1 or a $100 fee will be incurred. Please review the “Letter from the Health Center” which is included in the “Important Information” section of SFO.
